Coordinated adult primary care, mental health, chronic disease management, and home-based medicine for patients with complex medical needs.
Led by Dr. Charles Berko, MD, MPH, FACP and Dr. Oretha Sogbie, DNP, AGACNP-BC, PMHNP-BC, Pragmatic Health Services provides coordinated adult medical and behavioral healthcare for patients with complex needs.
We see patients in our Bel Air office, in their homes, and in assisted living and skilled nursing facilities throughout Harford County. Care is coordinated across all four service lines to improve continuity and reduce fragmentation of care.
Comprehensive adult primary care focused on prevention, chronic disease management, medication oversight, and continuity of care for medically complex adults.
For patients living with two or more chronic conditions — diabetes, heart disease, COPD, hypertension, kidney disease, obesity — coordinating across specialists, medications, and lab results is a job in itself. We manage that job for you.
Psychiatric care for adults — evaluation, diagnosis, medication management, and ongoing follow-up. We treat the behavioral health symptoms that often accompany chronic medical illness, so you don’t have to coordinate between two clinicians.
For patients who can’t easily come to us — or who shouldn’t have to — we come to you. House calls, ALF and SNF rounding, and structured follow-up after hospital discharge throughout Harford County.
If this sounds like you or someone you love, we’re accepting new patients. Timely appointments available.
