How we work

Led by Dr. Charles Berko, MD, MPH, FACP and Dr. Oretha Sogbie, DNP, AGACNP-BC, PMHNP-BC, Pragmatic Health Services provides coordinated adult medical and behavioral healthcare for patients with complex needs.

We see patients in our Bel Air office, in their homes, and in assisted living and skilled nursing facilities throughout Harford County. Care is coordinated across all four service lines to improve continuity and reduce fragmentation of care.

i.

Primary Care

Comprehensive adult primary care focused on prevention, chronic disease management, medication oversight, and continuity of care for medically complex adults.

What’s included
  • Annual physicals and preventive screenings
  • Longitudinal management of adult chronic conditions
  • Medication review and ongoing oversight
  • Specialist coordination and care navigation
ii.

Chronic Care Management

For patients living with two or more chronic conditions — diabetes, heart disease, COPD, hypertension, kidney disease, obesity — coordinating across specialists, medications, and lab results is a job in itself. We manage that job for you.

What’s included
  • Coordinated care planning across all your conditions
  • Medicare CCM enrollment for patients who qualify
  • Cardiometabolic risk reduction
  • Medication review and reconciliation
iii.

Mental Health

Psychiatric care for adults — evaluation, diagnosis, medication management, and ongoing follow-up. We treat the behavioral health symptoms that often accompany chronic medical illness, so you don’t have to coordinate between two clinicians.

What’s included
  • Psychiatric evaluation and diagnosis
  • Medication management for adult mental health conditions
  • Care for depression, anxiety, insomnia, and adjustment disorders
  • Behavioral health support for patients with chronic medical illness
iv.

Home & Facility-Based Care

For patients who can’t easily come to us — or who shouldn’t have to — we come to you. House calls, ALF and SNF rounding, and structured follow-up after hospital discharge throughout Harford County.

What’s included
  • House calls for homebound and mobility-limited patients
  • Rounding at participating ALFs and SNFs
  • Transitional Care Management after hospital discharge
  • On-site evaluation, treatment, and coordination
